Guidelines for Applying for Medicare Subsidies to Kaohsiung Municipal United Hospital

Medicare Subsidy for Hospitalization

Based on an assessment by the Hospital, medicare subsidies of varied amounts will be granted to applicants.

The patient should have the National Health Insurance, be visiting the outpatient/ emergency medical services (or be hospitalized), and have financial difficulties.

1. The Poverty Certificate issued by the village chief in the patient’s household registration residence.
2. The Certificate of Diagnosis issued by the physician of the Hospital.
3. A photocopy of the household certificate.
4. The Certificate of Household Income issued by the National Tax Administration.
5. The Certificate of Real Estate Ownership issued by the National Tax Administration.
6. Other relevant documents.

Subsidy for Hiring Care Givers

Based on an assessment by the Hospital, medicare subsidies of varied amounts will be granted to applicants (a maximum of NT$ 1,500/person/day).

For the patient who has no family members or is out of contact with his/her family members, and has to be hospitalized, his/her attending physician judges that a care giver is required to take care of the patient during hospitalization, and the patient cannot afford to hire a care giver due to financial difficulties.

1. The Poverty Certificate issued by the village chief in the patient’s household registration residence.
2. The Certificate of Diagnosis issued by the physician of the Hospital.
3. A photocopy of the household certificate.
4. The Certificate of Household Income issued by the National Tax Administration.
5. The Certificate of Real Estate Ownership issued by the National Tax Administration.
6. Other relevant documents.

2009 Immediate Care & Emergency Relief

NT$ 10,000~30,000 for Emergency Relief

This Relief is applicable to the following situations:
1. The livelihood is in a difficult position because the principal family supporter dies, disappears, suffers from a severe disease/injury, is unemployed, etc.
2. The livelihood is in difficult position due to other unforeseen happenings

Citizens in emergency need, or his/her relatives, neighbors, communities, schools, relevant agencies, institutions, and organization, may submit an application form/report (see attachment 1) and apply for emergency relief or report to:
1. Offices of Village Chief
2. Township/City/District Offices
3. Municipalities and County (City) Governments
